Online Forms:
You can now submit financial request forms online! Simply click on the form you need below, fill it out completely, then click submit. Your completed form will be automatically sent to the state treasurer. If you prefer to mail a hardcopy form, printable forms are available in your board manual.
Reimbursement Request Form: Used to request reimbursement for authorized expenses. Receipts must be provided.
Payment Authorization Form: Used to request payment to a vendor/provider for authorized expenses. Bills/invoices must be provided.
Trip Expense Form: Used to request reimbursement for authorized travel. Receipts must be provided.
Journal Entry Payment Authorization Form: Used to request that funds be moved from one NSCDA-SC account to another. An example would be a gift from a Town Committee to The Colonial Council.
